Sittingbourne Primary Care Network
Primary Care Networks (PCNs) are a key part of the NHS Long Term Plan. General Practices are part of a PCN network, typically covering 30,000 to 50,000 patients. These networks will provide the structure and funding for services to be developed locally, in response to the needs of patients they serve. Sittingbourne PCN covers a registered population of just over 57,000 patients.
PCN Vision
Sittingbourne Healthcare Matters: We are committed to a future where healthcare is transformed through innovation, teamwork, and a focus on prevention. Working with partners, we will make Sittingbourne a thriving community where everyone is empowered to stay well.
PCN Purpose
Our purpose is to collaborate using a patient centred approach, alleviating health inequalities to improve the health and wellbeing of Sittingbourne.
